Fascinating read. I read about 40 pages and skimmed the others.
It would seem the main culprits are subpar equipment (all site failures were attributed to 3 brands, one is no longer in business) and FIRMWARE UPDATES needed but not done.

Separately I find their lack of SMA and Schneider gear correlative to their failures.

They also cite poor commissioning practices and installs not representing case studies or plans
